TradMaD Camp Covid Attestation - 2023

By attending TradMaD, I attest that, as far as I know:

• Neither I nor any of my household have COVID-19 or symptoms of COVID-19, including fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, and diarrhea
  
  
• Neither I nor any of my household have had those symptoms in the past 5 days
  
  
• Neither I nor any of my household have tested positive for COVID-19 in the past 5 days
  
  
• I have not been in close contact with anyone confirmed to have COVID-19 infection or suspected COVID-19 infection based on symptoms in the past 5 days.
  
  
If I develop any suspected COVID-19 symptoms, I will immediately take a rapid antigen test and contact session organizers

If I learn, while at Camp, that I was in close contact with an individual confirmed or suspected to have COVID-19 prior to my arrival, I will immediately contact session organizers


• If I am a close contact to a camper who tests positive while at Camp, I may be asked to eat separately during meals and wear an N95/KN95 when interacting with others
